Telephone Listing Changes

Please report any listing changes on the listing change form so that we can update names and phone numbers in our database.

Listing Change Form Information

It is very important to consult the latest copy of your monthly telephone bill while you are filling out the Listing Change form.

The purpose of the Listing Change form is to change any incorrect listings for the on-line directory used by the Operators and for bill clarification, or to change any listings that need to be altered due to an employee leaving or joining the University.

Mail or fax the completed form and extension summary changes to Marie Robinson, Telecom Systems Office, 101 University Dr. Suite B1, Amherst, MA 01002, or fax it to 5-1798.

Note: It is also very helpful if you include a copy of your current Extension Summary Report along with the form.
