2004 Telephone Directory cards going out with Sept. 26 paychecks
Telephone Directory Update Cards (yellow cards) will be distributed
to employees with paychecks on Sept. 26, 2003. Every employee on the
UMass payroll as of Thursday, September 18, should have received a
card (excluding students, graduate assistants and fellows). The cards
will be collected and used by Telecommunication Services to publish
the 2004 Faculty and Staff Telephone Directory. Anyone who does not
receive a card should call 7-3188. All cards should be signed,
updated if necessary, and returned by October 10, 2003 to the
Faculty/Staff Directory, Telecommunications, 101 University Drive,
Suite B1, Amherst, MA 01002. Updates received after October 10 may
not be included in the 2004 directory.
Changes to the Directory Update Card are made in the following UMass
directories and files, with exceptions noted below: The 2004 Faculty
& Staff Directory (printed), the online directory (PeopleFinder), the
SIS database (SPIRE, student transcripts), the Telecom TFMS database
(telephone bills and 911 directory), and the Human Resources file
(paychecks and W2 forms).
NAME - Changes to the name field will not be made in the Human
Resources file (paychecks and W2 forms). To change your name with HR,
you need to provide a copy of your social security card with your new
name, and fill out a "Name Change Form," which you can get at the HR
Employee Service Center, 325 Whitmore Administration Building
(5-6110).
EMAIL ADDRESS - Only official UMass email addresses are listed in
employee directories (use this card to correct mistakes). An official
UMass email address ends with "umass.edu," or "umassp.edu."
Exceptions to this policy include retired faculty and staff, and
employees at certain off-campus but related entities (e.g., Mass
Ventures, or the Newman Center). To change your official email
address, send a request to directory@oit.umass.edu.
JOB TITLE - You may correct your job title only with a supervisor
signature (for the printed and online directories). To request a
change to your HR title, your supervisor must fill out and submit a
"Personnel Action Form" (www.umass.edu/humres/PATemplate.pdf). UMass
directories default to "working titles" for professional staff, and
"state titles" for faculty and classified staff (please note that
classified staff may indicate a "working title").
HOME INFO LISTED? - If the box reads, "YES," then your home address
and telephone number will be published in the employee directories,
and is considered public information. If the box reads, "NO," then
your home information is restricted, except where provided by law. If
you wish to add or discontinue this restriction in time for the 2004
printed directory, you need to file a new "Directive Form" with the
HR Employee Service Center by Nov. 7, 2003. This form is available at
www.umass.edu/humres/fipaform.pdf. You cannot change this information
with the Directory Update Card.
SIGNATURE AND DATE - Please sign and date the card (whether there are
changes or not), and return it to Telecommunications by Oct. 10, 2003.
