Tug Rep Area Main
This forms area has been created to aid TUG Reps in providing all the information necessary
to process a telephone request. Links within certain forms have been provided to offer additional information.
For auditing purposes, Telecom requires a TUG Rep signature (and with Project/Grant accounts, an original principal
investigator signature) to process a work order request. Therefore, our online forms must be completed, printed and signed, then FAXed or mailed
to the appropriate Telecom Rep at 101 University Drive, Suite B1.

Customer Service (TUG Reps)
- Install/Deinstall Telephone Service
Use this form to install new service or remove existing service.
- Move/Add/Change/Upgrade/Downgrade Telephone Service
Use this form to move, add, or change service on an existing telephone.
- Activate/Deactivate Networking Service
Use this form to activate or deactivate a network connection for your computer.
- Suspend/Unsuspend Telephone Service
Use this form to suspend or unsuspend your telephone service. If you have a telephone that will not be used for
several months or more, you can suspend your service for an indeterminate amount of time. The cost to
suspend service is $15. There is an additional $15 charge to un-suspend service. Depending upon the monthly
charges for the phone, it may be more economical to suspend it for a period of time rather than to de-install and then
re-install at a later date.
- TUG Representative Change Form
This form must be fully completed and signed by the old TUG Rep or authorized Department Head to change your departmental TUG Representative in our database.
- Satisfaction Survey
Use this form to report changes you would like to see, areas needing improvement, or just good service that you would like
to acknowledge. Our committment is to provide excellent customer service and we rely on constructive feedback to keep in
touch with your needs and concerns.

- Listing Changes
Please report any listing changes on the Listing Change Form so that we can update names and phone numbers in our database. Here are some

- Expense Transfers
If you are moving around within the University you need to transfer any telecommunications expenses from your old speed type to your new speed type number. Here are some

- Long Distance Authorization Code
A long distance authorization code allows faculty and staff members access to long distance dialing while on campus. Here are some
instructions for this form.
- Calling Card Request
Faculty and staff members who are required to make business calls from telephones off campus need to request a Qwest Calling Card. Here are some
instructions for this form.
- Calling Card Deactivation
Use this form to deactivate your calling card. Here are some
instructions for this form.
